Cores Comparative Effectiveness Research on Cancer in Texas (CERCIT)

Administrative Core Director: James S. Goodwin, MD

James S. Goodwin, MSThe Administrative Core of CERCIT will provide overall leadership and direction for all the activities of CERCIT projects and cores, including coordinating and integrating their functions and promoting scientific coherence. It will track and evaluate all projects and cores, reallocating resources as appropriate, to promote quality, productivity and efficiency in all CERCIT activities. It will organize regular research and administrative meetings of the CERCIT investigators and trainees and the meetings of the National Advisory Committee and Texas Stakeholder Committee. The Administrative Core will incorporate the dissemination activities that were in the Dissemination Core in the initial CERCIT. As in the initial CERCIT, the Executive Committee will consist of the project and core directors and a member of the Texas Cancer Registry. The Executive Committee will meet monthly. The CERCIT administrator, Dr. Eschbach (who directs dissemination), the CERCIT web master and a representative from the Texas Area Health Education Center (AHEC) also attend the meeting.

Data Analysis Management Core Director: Yong Fang Kuo, PhD

Yong Fang Kuo, PhD The Data Management and Analysis Core (DMAC) has three goals. First, it constructs and maintains data resources to support research on cancer care and outcomes in the state of Texas. Second, it supports the Administrative Core in dissemination of data, and the Training Core through biostatistics mentoring. Third, it is a biostatistics hub providing analytical consultation to the 4 CERCIT research projects and developing new analytical approaches for comparative effectiveness research on cancer care and outcomes. The DMAC will extend four CERCIT data bases to the years 2014-2018: 1) TCR linked Medicare data (TCR-Medicare); 2) TCR linked Medicaid data (TCR-Medicaid); 3) Texas Medicare 100%; and 4) Texas Medicaid 100%. The Core will continue to link supplemental databases to each of these four major data files. The DMAC will coordinate with other CERCIT cores for dissemination, training and survey data processing. This includes collaboration with the Survey Core to provide sampling frames and data linkage between surveys and TCR-Medicare and between surveys and TCR-Medicaid. The DMAC will adopt and develop innovative analytical approaches to support CERCIT research projects, including methods to compare effectiveness across different types of tests, treatments, and models of care; use of social network models to study providers in cancer screening and cancer treatment; and data mining to generate hypotheses on chemotherapy associated toxicities.

The institutional review boards at the University of Texas Medical Branch at Galveston and the Texas Department of State Health Services approved the study as did the privacy review board of the Centers for Medicare and Medicaid Services.

Data Use Agreement Protocol

Data Source: Data from the Texas Cancer Registry (TCR)–Medicare linked database were used for the analysis. The Texas Cancer Registry–Medicare linked database is a linkage of two large population-based sources of data, performed under the guidance of the National Cancer Institute (NCI) 1, the Texas Cancer Registry 2 and the Medicare claims data collected by the Centers for Medicare and Medicaid Services.3 This data set provides detailed information about elderly adults with cancer in Texas. Approximately 98% of all people aged 65 and older in TCR are matched with Medicare enrollment and claims files. TCR collects and provides information on participant demographics, cancer prevalence, cancer incidence, stage of disease, first course of therapy, and survival. The Medicare claims data include information on hospital stays, physician services, and hospital outpatient visits. Data use agreements have been signed with both data providers. The data used in this study include cancer patients diagnosed with __________ cancer between 1995 and 2007 and their Medicare claims through 2009.

Survey Core Director: Susan K. Peterson, PhD, MPH

Susan K. Peterson, PhD, MPH The Survey Core will support the design, development, and implementation of survey and qualitative research for all projects in the proposed application. To address limitations or gaps in currently available measures of patient-reported outcomes, Projects 1 and 3 will use a qualitative approach to develop and/ or adapt patient-reported outcome measures. Development of new items and measures will be driven by the results of qualitative research to inform their content. Measures not available in Spanish will be translated into the Spanish language to accommodate prospective participants for whom it is their primary or preferred language. The core will employ a systematic set of field procedures to maximize response rates that includes multiple waves of follow-up procedures for non-respondents.
